Clinical Data Specialist/Home Health Aide

Iowa Home Care is seeking a dependable, organized, and compassionate individual to join our team in a dual-role position as a Clinical Data Specialist / Home Health Aide. This full-time role combines office-based clinical data responsibilities with direct patient care in the home setting.

The ideal candidate is detail-oriented, comfortable working with electronic medical records, and passionate about providing excellent care and support to patients and staff.

Essential Duties & Responsibilities

Clinical Data Specialist Responsibilities

• Enter, review, and maintain accurate patient information in electronic medical records (EMR)
• Assist with chart audits and ensure documentation compliance
• Review physician orders, visit notes, and care documentation for accuracy and completeness
• Support scheduling and coordination of clinical services
• Communicate with clinical staff regarding missing or incomplete documentation
• Assist with tracking orders, signatures, and compliance requirements
• Maintain confidentiality and HIPAA compliance at all times
• Provide administrative support to the clinical management team

Home Health Aide Responsibilities

• Provide personal care assistance to patients in their homes under the direction of a nurse
• Assist with bathing, grooming, dressing, toileting, and mobility
• Observe and report patient condition changes to supervising staff
• Maintain a clean, safe, and supportive environment for patients
• Document aide visits and patient care accurately and timely
• Build positive relationships with patients and families through compassionate care
